How do I become a patient here?
We are always accepting new patients. Call our office at 641.255.2688. New patients may also self schedule online through our patient portal. Use the “Schedule Appointment” button to book.
Who can be treated here?
Everyone—men, women, children, infants, elderly. We accept patients of all ages.
Are you currently accepting obstetrics patients?
Yes, we are! Sincera Health is currently welcoming new obstetric patients.
Do you offer same-day appointments?
Yes. If our schedule allows, we can take same-day appointments. Self-scheduling is coming soon. To schedule your appointment, please call our office at 641-255-2688.
Do you take walk-ins?
No, but Sincera Urgent Care does! Walk on in or schedule online at sinceraurgentcare.com. Urgent care is open Monday - Friday (7 a.m. - 7 p.m.), Saturday (7 a.m. - 1 p.m.), and Sunday (1 - 7 p.m.)
I’m currently seeing another primary care provider and would like to switch. How can I transfer my medical records?
Our team can assist you with the transfer of your records. At check-in on your first visit, you will be asked for a signature to initiate the transfer of your medical records to our practice.
Insurance & Self Payment
Do you take my insurance?
Sincera accepts most major insurance plans, including Blue Cross/Blue Shield, HealthPartners, Aetna, Iowa Total Care, Midlands Choice (Cigna), Medicare, Medicaid, Molina Healthcare, Amerigroup, First Health Network, Boomy Health, Sanford Health Plan, United Healthcare Community Plan, and others.
To check if Sincera Health or Sincera Urgent Care is in-network for your specific plan, please visit your insurance company’s website, where they typically list in-network providers.

I don’t have health insurance. Can I still visit Sincera Health?
Yes! We offer self-pay pricing for patients without health insurance.
How much does a typical visit cost at Sincera Health?
The cost of a visit varies based on the services you receive and the specifics of your health insurance plan. Sincera accepts most major insurance plans to help patients avoid out-of-network costs and also offers affordable self-pay pricing for those without insurance or those who wish to pay for their visit without using their insurance.

Services
Do you offer physicals for work, school, or sports?
We do offer well-child, wellness, school, and sports physicals. We do not offer occupational health or employee physicals at this time.
Do you have lab testing and imaging on-site?
Yes, Sincera Health offers on-site lab and imaging services.
Urgent Care
Do you still see urgent care patients?
Yes! We are still seeing patients at Sincera Urgent Care. Spot reservation and more information about Sincera Urgent Care is available here.
I have an urgent care illness/injury, but I would like to see Dr. Carlstone. May I make an appointment to be seen at Sincera Health instead of Sincera Urgent Care?
Yes. If you are an established part of Sincera Health’s practice and Dr. Carlstone has availability, you may choose to see him for any urgent care needs.
Results, Prescriptions & Referrals
How will I receive test results?
You’ll receive your test results through our secure patient portal as soon as they’re available. In some cases, your provider or health team may also call you to review the results or discuss next steps.
How do I request a prescription refill?
In most cases as an established Sincera patient, you should initiate a refill through your pharmacy. If you haven’t yet seen Sincera Health, a visit is important to ensure your medication is still appropriate and safe for your current health condition.
What if I need to see a specialist?
If your condition requires the attention of a specialist, we are happy to make a referral.
